AHD Commodity Calculator
People living with advanced HIV disease (AHD) remain at high risk of opportunistic infections (OIs), including cryptococcal meningitis (CM), tuberculosis (TB), and increasingly, histoplasmosis. Ensuring countries can accurately forecast and procure the diagnostics and treatments needed to manage these conditions is critical to maintaining uninterrupted access to care and reducing preventable mortality. However, many national programs have relied on different assumptions and quantification approaches, making it difficult to compare forecasts, identify commodity gaps, and aggregate demand across countries.
To help address this gap, CHAI worked with the Global Fund to develop the AHD Commodity Calculator to help countries optimize quantification and forecasting of essential AHD commodities. The AHD Commodity Calculator enables national programs to input country-specific data on AHD cohorts, coverage targets, cryptococcal antigen (CrAg) positivity rates, and preferred treatment regimens, with commodities automatically quantified across key areas of AHD care.

To support adoption, CHAI has also developed a step-by-step video walkthrough that explains how to complete each input tab, apply country-specific assumptions, and interpret the final commodity and cost outputs.
These updates were validated by early adopted countries supported by Global Fund, Market Access Africa, and Aurum, and will be shared with the Global Fund and CQUIN network to support countries undertaking Grant Cycle 8 supply planning and forecasting activities. The calculator and accompanying guidance are also publicly available for use by any country program or implementing partner.
By enabling countries to quantify histoplasmosis diagnostics and treatment alongside existing AHD commodities, the calculator supports a more complete and standardized approach to procurement planning. Ultimately, the tool aims to reduce the risk of stockouts, strengthen supply continuity, and help ensure patients can access the full continuum of AHD care.
Through the generous support of Unitaid, the Clinton Health Access Initiative (CHAI)-led THRIVE Project is enabling access to critical prevention, screening, and treatment commodities for advanced HIV disease to dramatically reduce mortality among adults and children living with HIV.
